NDIS Record Keeping: Documentation by Support Type

The NDIS requires specific documentation for each support type you deliver. This guide maps every support category to its required documentation — based directly on the NDIS Provider Toolkit.

Minimum Documentation Requirements

For every claim, you must record at minimum: the participant's name, dates of support, total quantity delivered each session, and the support type. Include the participant's NDIS number whenever possible.

Support Log

Records the support type (including ratio/intensity) and must be signed by the participant, nominee or carer confirming receipt of each support instance.

Roster

Records activities engaged in, staff-to-participant ratios, and that the participant attended as scheduled.

Case Note

Documents the activities engaged in and how they relate to the specific support item and the participant's goals.

Service Agreement

Records the schedule, cost, nature and quality of supports to be provided and the expected outcomes.

Capital supports (continence products, HEN, assistive technology, vehicle/home modifications) should be invoiced. Invoices, assessments and reports with minimum identifying info may not require additional evidence of quantity.

Wherever possible, logs should be signed by the participant, parent/guardian, nominee or carer as confirmation support was delivered as claimed.

Documentation Required by Support Type

* Where support involves writing a report, additional documentation is not required if participant and session info is in the report.
** If a report results from a team meeting, the report/case note should document all team members present.
*** Could use support log or group roster, but should have an accompanying case note for each participant.

What Makes a Good Case Note?

An effective case note should document:

  • The activities engaged in during the session
  • How those activities relate to the specific support item being claimed
  • Progress made by the participant
  • Plans for future sessions
  • Participant goals being worked toward

Case notes should be completed as close to the time of support delivery as possible — ideally within 24 hours.
